Your mobile app and manual claims

Mobile app claims

Submit your Extras claims with the free RT Health mobile claiming app. 

You can download the mobile app from the Apple store for iOS mobile devices and the Google Play Store for Android mobile devices.

Claiming is simple, just follow these three easy steps:

  1. Select “make a claim”
  2. Upload or take a photo of the receipt
  3. Select Submit

The mobile app has been designed to make it easier for members to submit a claim, view your account details and view your claims history.

To login to the mobile app you will need your membership number and Password (which you use for your Member Self Service login). If it's your first time logging in, you will be required to register, which can be done on the login screen.

To reset the password, select forgot password on the login screen, which will take you to the password recovery screen.

The mobile app is designed to submit Extras claims, such as but not limited to pharmacy claims, physiotherapy claims and massage therapy claims.

You are no longer required to submit a claim form if you use the mobile app.

The photo of each receipt needs to be taken in high resolution from your phone camera.

If you have more than three images to attach, please submit separate claims. Each claim submission can hold a maximum of three attachments.

The mobile app usually takes around 30-40 seconds to load. However, if there are a lot of users, it may take more than a minute to download the mobile app. If you experience issues with the mobile app not starting up, you will need to restart your phone, which should resolve your issue.

Manual claims

Make a claim by printing off the following claim form and sending it to us with a copy of your receipt.

Before lodging your claim, please check that your receipt contains the following details:

  • Full name of the person who received the product, service or treatment.
  • Date of your service/treatment or the date the product was purchased.
  • Name of the healthcare practitioner who provided the product, service or treatment.
  • Provider number of the healthcare practitioner.
  • Address and contact number of the healthcare practitioner.

Please remember

  • If your healthcare provider gives you an unpaid invoice we’ll pay the benefits into the Policyholder’s nominated bank account and they’ll be responsible for making sure your health service provider is paid.
  • All claims submitted must be less than two years from the date of service.
  • If you are sending in a claim for health aids, orthotics, orthopaedic shoes or contraceptives, you will need to include a letter from your prescribing practitioner detailing the medical reason you require the aid.
  • For inpatient medical claims we require you to send in the Medicare statement unless your invoice tells you to send it directly to your fund.
  • If you require your original receipt, please send us a copy as we do not send originals back.

Claim form

Use this form to make all medical and Extras claims (where HICAPS is unavailable). Please print, complete, sign and then send it to us by email, fax or snail mail.

Gym Benefits Authorisation and Claim

Use this form to claim the cost of your gym membership if your extras cover includes a benefit for this service.

Direct Debit Form

Use this form to set up an automatic direct debit from your bank account or credit card. Please print, complete, sign and then send it to us by email, fax or snail mail.

Third Party Authority

Use this form to nominate someone else to make changes, ask about claims and generally manage your membership in case you're away or unable to do it yourself.

Partner Authority

As the main member of a couple/family membership, use this form to give your nominated partner full authority (this excludes the ability to cancel or suspend your membership).

Direct Debit Service Agreement

Salary Deduction

Would you like to pay for your health cover directly out of your salary? If your employer has a salary deduction plan, use this form to set this up. If they don't, ask payroll to give us a call.

Dependant Student

Your kids are covered by your family membership until they turn 22. After that, they can stay on your cover at no extra cost up until the age of 31 if they qualify as a 'student dependant'.

Rebate Tier Nomination

If you're not currently receiving the government rebate as a reduction in your contributions, or need to change your rebate level, you can change your rebate option here.

Suspend Cover

Travelling overseas? You can't take your health cover with you, but you may be able to suspend it while you're away. See all of the terms and conditions on page 2 of this form.
